Smardii is tackling a global issue which is the exponential increase of the elderly population combined with the lack of nurses to take care of them. 

Nursing homes have been struggling with poor staffing ratios, increased costs and liabilities, lack of resources (financial, technological, manpower), and poor patient outcomes for decades. We all know that the COVID-19 pandemic has even worsened the situation.

That’s where our sensor-based, wearable solution comes in -  transforming any adult incontinence protection into a smart garment that continuously monitors and detects key parameters.

Smardii is currently in the pre-revenue stage.

The Problem

The Long-Term Care Industry is Facing Several Challenges

Incontinence is the starting point of many critical issues in eldercare. Besides losing their dignity, they also become more prone to bedsores and other skin Infections, aggravated urinary tract infections, and lack of sleep. These have proven to negatively impact falls, exacerbation of dementia, and severe cognitive illnesses such as Alzheimer’s

In short: quality of life goes down, while healthcare costs go up.

Unless they have a loved one in a nursing home, most people don’t know that elderlies living in a nursing home are sometimes woken up 4-5 times a night in order to check on their diapers. Especially at an old age, lack of sleep is a serious issue that has many consequences on their physical health and their mental well-being. Imagine being woken up 4-5 times a night! 

In addition, as a result of the pandemic, they are now being manually checked more often in an effort to flag any potential risk of infection or health degradation that could lead to their hospitalization and potentially expose them to COVID-19 cases. Now, operators have to multiply unnecessary contacts by manually checking on residents every 2-3 hours on average. Without technology, there is no other solution for caregivers.

Products related to incontinence already represent more than a third of nursing homes’ spending and with these new protocols, nursing homes spent more time and more money than ever to monitor their residents. This is not economical, not efficient, and not patient-centric.

Despite this, it still doesn’t guarantee that residents will not be in a soiled environment for a long time. If a patient wets or soils their garment shortly after being manually checked, they could remain in that state for several hours.

Furthermore, over the years, the lack of skilled nursing workers has led to a degradation in the quality of care at skilled nursing facilities. The staffing levels at nursing homes have not kept up with the rise in admissions, which leads to increased strain on operations at such facilities across the country producing a potential rise in complaints of abuse, neglect, and overall poor quality of care.

When trying to put a dollar amount on some of these issues, we learn that:

  • The exponential increase of the elderly population accompanied by the shortage of nurses and medical staff increases risks and liabilities for healthcare facilities and affects the quality of care

  • Increase in the cost of eldercare due to lack of innovation and lack of automation for certain tasks 

  • Cost of UTIs is $1.6 Billion per year in the U.S.

  • Cost of Falls are $50 Billion per year and 3 deaths per hour in the U.S. for people aged 65+

  • Covid-19 has put an additional strain on healthcare operators who are now forced to find solutions or they may close down.

The Solution

Smardii. Better Care

Smardii is an IoMT (Internet of Medical Things) medical device that, when attached to any adult brief, is able to detect an array of body functions like urine, stool, and body positioning for fall detection and pressure ulcers monitoring. This is the first generation of our product, already in use at nursing homes, and FDA-compliant.

And, we are on our way to include geolocation, body temperature reading, and real-time urinalysis in an upcoming version (Smardii Gen2, a.k.a. Smardii Plus).

Aside from the obvious quality of care and life improvement for patients, we are also eliminating tedious manual processes for medical staff. This means our solution offers immediate and substantial cost savings for healthcare facilities.

Plus, our technology is also building an entire health history for patients. Healthcare facilities can show clear evidence of the quality of care given or reported to their regulators.

This becomes very important where neglect lawsuits can cost hundreds of thousands (sometimes millions) of dollars to nursing homes. Bedsores being evidence of neglect is difficult to overcome before a court of law, when the caregivers cannot produce compelling evidence. Smardii can help bridge that gap.


How it works

 Intelligent Patient Monitoring

Smardii is made up of the following: 

  • Disposable (single use) Sensor Strips: With an adhesive backing, the sensor strip is attached to the inside of an adult incontinence garment, either a diaper or pull-up brief.

  • The “Puck” - A reusable, rechargeable, and waterproof communication device: The Puck clips onto the outside of the garment and attaches to a protruding tab on the sensor strip. This is the “brains” of the Smardii system. The Puck takes information from the sensor strip and sends it to the Cloud, via a gateway.

  • LoRa Connectivity Gateway: The gateway is simply plugged into a standard electrical outlet, takes information from active Pucks, and sends it to the Cloud.

  • The Smardii Portal: a web-based software that pulls down information from the Cloud. Alerts, preset notifications, and other information can be displayed in graphical or numerical formats on any smart device.

From there all you have to do is:

  1. Plug the Gateway in, and after 3 to 5 minutes, the entire system will go live and automatically connect to our Cloud servers.

  2. Charge the pucks, four hours each. Batteries last for 90 hours.

  3. With a nurse practitioner's help, peel off the adhesive backing the sensor strip and apply it into whatever brand or type of incontinence protection is being used.

  4. Put on the pull-up, with the sensor strip now in it. The sensor strip itself is not touching the skin since it is protected by a biocompatible mesh material. It is disposable, very flexible, very soft, and residents do not notice any difference.

  5. The puck goes outside of the protection and physically clamps onto the sensor strip. After a few seconds, the data will start flowing from the Puck to the Cloud.


Our current version detects the presence of urine, stool, and analyzes body positioning. The key functionalities are:

The business model is fairly simple because our customers, who are facing many challenges at the moment, need things to be as simple as possible.

That’s why we have a monthly recurring Software as a Service and Hardware as a Service so customers pay a single price below $99 per person all-in.

When it comes to Medicare, we see it as a great incentive for healthcare operators to adopt Smardii, but we believe our solution is already compelling enough. However, with reimbursement from Medicare, customers could possibly get Smardii fully paid for, or even make a profit. Based on a recent study conducted by a third-party reimbursement expert, we believe Smardii is eligible for several CPT Medicare reimbursement codes.

Over time, Smardii will be gathering valuable data, which we intend to monetize while complying with all relevant laws and regulations such as HIPAA, GDPR, and the like.

Our solution is auto-provisioning, which means there is no equipment installation needed. Even during the peak of the pandemic, we could get our system running with no effort. 

Our solution uses a private network, which means that we do not need to access our customer’s WiFi. This gives them a lot of comfort in terms of cybersecurity, and we do not saturate their WiFi bandwidth.

The Vision

Smardii in testing collaboration at the Fraunhofer Institute via Vivai Germany

Smardii is currently collaborating with Germany-based company VIVAI ( & on the testing of the Smardii system in view of a possible pilot roll-out in Germany. The actual testing will be performed in the coming weeks by the Fraunhofer Institute (

The Fraunhofer-Gesellschaft, headquartered in Germany, is the world’s leading applied research organization. With its focus on developing key technologies that are vital for the future and enabling the commercial exploitation of this work by business and industry, Fraunhofer plays a central role in the innovation process.

Vivai is one of Germany’s pioneers in Internet of Things as applied to Elder Care. After (unsuccessfully) evaluating several competing products, Vivai has decided to collaborate with us and Fraunhofer in order to possibly bring Smardii to the German/Austrian/Swiss markets. This comes as a critical need to provide an accurate and efficient stool detection solution to eldercare providers in that region which, except for Smardii, did not exist to date.

Dear Shareholders, 

It is with great pleasure that Smardii received earlier this week a Notice of Allowance from the USPTO for one of its Patent Applications.

This patent, also filed globally through PCT applications, is instrumental in our continued growth and strategy towards building a digital health device able to improve patients' outcome. Among other things, it protects how we can detect and differentiate between urine and stool which is, as you know, one of Smardii's key differentiators.

Hopefully more patent allowances to come!
